There are several key forms you may need to access throughout your Promise journey. You can find most of them below!
Scholarship Application
Apply to certify your eligibility for The Kalamazoo Promise and your eligibility percentage. Students may submit their application once they have begun their senior year of high school.

Scholarship Acceptance Form (SAF)
Submit a Scholarship Acceptance Form when you have decided where you want to use your Promise and have a college ID number.
You MUST have a Scholarship Acceptance Form on file in order for your Promise funds to be sent to your chosen institution.

NOTE: You MUST fill out an SAF for any school you would like to use your Promise award at; You may only use your Promise award at 1 school per semester.
Initial Eligibility Appeal
Submit an appeal for your Kalamazoo Promise Eligibility. Students may only submit one eligibility appeal, and the student must have started their senior year of high school. You can provide information about any extenuating circumstances as part of your application for Promise eligibility.
Please contact your student advocate to begin your appeal process.
Extended Probation / Reinstatement Request
If your scholarship has been suspended, you may submit a reinstatement request after earning the appropriate number of credits without using your Promise funding. You will need to submit transcripts showing your earned credits. See the Reinstatement Request for more details.
Alternatively, you can work with your student advocate to create a success plan and request extended probation. Please reach out to your student advocate directly to discuss your options.

145 Credit Limit Waiver
If your degree requires more than 145 credit hours* you may be able to extend your benefits by completing a 145 Credit Limit Waiver and returning it to your school’s registrar office for consideration.
* The Kalamazoo Promise will not cover additional credits due to dropped or failed classes or those required for a minor or a second major. The Promise counts all credits for which they have paid toward the 145 credit limit.

Active Military Service Extension Request
Promise eligibility may be extended beyond 10 years of high school graduation if a student has not used all 145 credits or not completed a bachelor degree and has had active military service. To complete this process please contact our Promise administrator by clicking the button below and submit a copy of your military service record form DD 214.
